if you're just starting out adzerk.com is pretty decent (and free) used it for a media buy last month and it did the job pretty well. or the sitescout adserver which has way more features than adzerk.
I've used adzerk, worked well in the past.
how does adzerk work exactly? i just signed up and no idea how to grab the javascript code and link my campaign. it says that i need to add something to the header of the page and put the tag in between divs? that seems complicated to hand out to an advertiser as they usually only have access to the location of the ads...
I tried AdZerk and also found it a bit weird to get a handle on. I think you need to go to plugins/addons and find the advertising services plugin and add it to your account, which then gives you the codes you're expecting. If I recall rightly when I first signed up it assumed I was a publisher only.
I've used OpenX for several years and I'm happy with it. Enterprise is $1,000/month if you want a dedicated server. Otherwise the community version is free and is pretty much the same.

Are people still using Open X? I'm looking for a basic adserver to do direct buys. Any recommendations for something easy to use and inexpensive?
You could try to use http://epom.com/, I've used it in the past when I was doing small media buys and I didn't want to add to the operations cost.
Now I use sitescout for direct media buys / e-mail banners
