I tried odesk and easy outsource and havent had much luck this time around. oDesk in the past has been good, but not so lately.
Need to hire 2 staff.
1) developer
Looking for a full time php/html/css/js developer with about 2 to 4 years experience, probably from eastern europe or Philippines. Good english a must, and happy to work as part of a team without detailed specifications (ie rapid development with lots of feedback approach)
2) graphics person
Need someone to create ads/banners/etc. Basic photoshop skills required, experience with ad/banner creation not essential but would be a nice extra.
What are the best places?

Nope but I suspect its the same as odesk.
Im probably missing a trick with odesk this time around. Didnt have too much trouble in the past but for some reason fail to find a good dev this time.
Someone suggested fiverr - which is an idea. Test someone for $5 bucks. If they do good work, offer them a perm gig.
Guru.com for the win. More expensive but much higher quality and more people with real skills. Make sure you search for people with good ratings and invite them to bid as opposed to opening it up to every "company" in India
Never heard of Guru.com, I'm going to have to check that out. I've relied almost completely on word of mouth referrals / FB for hiring.

For programming related...you pay for what you get. I usually pay a premium and go with a guy from Eastern Europe over an Indian / Filipino. Quality + communication's so much better.
Graphics wise Filipinos are good.
Odesk is good if you know how to use it. It's all about the filters. Minimum 4.5 feedback, 500+ hours, etc. I also give them an interview test so I can test their knowledge. From there I'll hire 5 people for the test run and give them the same assignment. I'm looking for quality of work, how fast they turned it around, how much instruction they needed. After 2 weeks I'll hire one permanently. Basically I split test them.
^^ I like your idea of hiring 5 people for a test, it's definitely worth taking that initial step.
I second paying a premium for eastern european or US guys. The percent of responses I've had from Indians and Pinos saying that they can do a job but actually don't have a clue what I'm asking or how to do it is around the 90's.
I've used programmers from many different countries: Nigeria, Poland, India, Brazil and Honduras.
The guy from honduras was extremely good, he was able to do all tasks in very short time and charged a low price, but then he started his own web design company and said he couldn't work for me full time anymore, only on a per project basis. So he refered me to an Indian friend that is my full time programmer at the moment, he works for me like 8 hours per day, is very smart and hard-working.
I've had bad experiences with some programmers before, but I was lucky to find this Indian guy, if it wasnt for the indication I would not have found anyone like him, because he is not active in odesk and those freelancer sites.
For graphics I don't have a full time designer, but I've hired US people for some projects only, and it is good to hire someone that has experience with affiliate marketing.
I've hired a couple of really talented Filipinos through EasyOutsource, but the quality like you say is a mixed bag. You've really got to interview, interrogate, hustle, stalk and damn near get in to bed searching for moles with anybody you take onboard.
I've never enjoyed Odesk, but maybe that was thanks to the mercenaries I ended up hiring.
Elance has always been a great option for finding skilled workers both home and abroad. I like the setup and there are some great workers on there. I do think the average gig on Elance can be overpriced though.
I don't have enough experience with Guru to comment there.
If you really want to get some 'diverse' candidates, try submitting a job post on Gumtree/CraigsList in the computer/programming section of your desired country. I found a good coder using that strategy before. Unfortunately I also got spammed to hell and ended up with email herpes. It's a double edged sword.
Odesk - only used for very basic admin type jobs.
Freelancer - pretty good for a range of services. Pay for what you get but their filtering options help when used correctly.
Elance - premium site, I've only ever used for design work.
for me its been word of mouth... hired 7 guys to do banners on odesk... for an adult affiliate we have... ALL 7 were crap.
As a couple people have mentioned some of these guys can be very easily swayed to give up the whole project you paid for.
Way back when I used to make posts that said I'd buy a copy of anyone elses completed projects no matter what it was. If you have code sitting around and want to make more money selling a second copy I'll pay cash today. I got almost an overwhelming response of 100's of people wanting to sell me your code you've paid for.
Not only is it hard to find good freelancers but honest ones that won't give away your secrets is even harder. Be cautious out there.
If you're going to build a full time, long term business I highly suggest hiring in house.
Im from the Philippines there's a lot of good developers and designers here and I might be able to help you. If you want the really good ones, post on jobstreet.com, jobsdb.com, gopinoy.com. Its a little bit more expensive to post jobs there but most applicants will be of higher quality because most of the people I know that look for jobs go straight to those sites and not on outsourcing sites like easy outsource or even onlinejobs.ph (which is decent, but still mostly hit & miss). Those are the sites in our country where people look for real jobs and not be an oursourced worker..
Some tasks need to be outsourced, it's simply not worth US manforce. The way I do it:
1) Hire 5 people
2) Send them all the same exact job
3) See who replies first, with the best quality
4) Fire the other 4
Now you have 1 employee. Rinse and repeat. EWA has 150ish US / EURO employees and they are necessary for certain tasks, and you do pay for what you get. I have certain tasks that cost too much money to automate, so I ended up actually opening an office in Bangladesh and doing the actual interviewing process with a manager out there. I have computer monitoring technology and team leaders, just like a traditional US business. And I save about 50% off from the per/hour cost listed on the sites above (neglabile, but it by far pays for the rent of the place & the manager cost).

Trustworthy, reliable, honest, no-bullshit staff are hard to come by when outsourcing. Especially when they are working for you remotely. But it just so happens that I have a solution. I'm also the owner of www.outleased.com. And what we do is, we lease out our full-time or part-time staff.

I've done the same strategy of hiring several at once, but using Fiverr.com
If they understand what you want they will ask more than $5, and that is a good sign. As the amateurs will say yes to stuff they clearly don't understand.
You could also try Onlinejobs.ph. We have plenty of programmers and developers for you to choose from.
I strongly suggest that you should also take a serious look at how you hire, not just where you hire. You want to attract the best people that would provide you the best value to work for you. And this has more to do with how you hire. Make a great job post and offer a great salary that's commiserate to the skill level you want from your workers. Be clear about what you want, what you expect from your workers, and what they can expect from you.
Check out gun.io for US/CA based programmers that work remotely of top caliber. This is not for cheap affiliate projects but i have had great results for legit code needing to be built.
