Quiz to increase opt-in rate + segment lists

I've read several times that a quiz on your LP increases opt-in AND allows you to segment your leads into different lists, so you can send targeted messages and increase conversion rates. (see below a post from maxentin about that, for example)

Sounds cool, but I can't find a tool to do this. The few I see online all seem to be reviewed poorly (buggy as hell, etc).

1. Is there a quality solution on the market?
2. Any experiences / suggestions about using this model - please share!

How to segment your market and capture leads at the same time? Best way to do this is using a quiz. Not any quiz - it has to be an assessment quiz related to the offer that's structured in a specific way. Here's how:
1. Questions in the quiz have to spark value-based curiosity. Your prospect should think "if I only knew the answer to that question it would really help me out”
2. The bait/reward for completing the quiz should be an automated question-by-question walk-through of the answers
3. Part of the quiz questions have to be questions that would allow you to segment your prospects into different lists
4. On the first part of the quiz you should tell them about the reward AND allow them to see the first few questions that they really want answers too.
5. At the end of the quiz tell them to sign up and you'll email them their assessment. At this point they'll be so curious about the answers that they'll really want to sign up. The opt-in rate when using a quiz is typically much higher than when using a regular squeeze page.

Kind of sounds like what you're looking for. I didn't buy it or try it. I have no idea how well it works.

Thanks Luscious. Looks pretty good. I found a "shareware" version too, not sure if it's ok to post the link here - mods, let me know.

Still looking for more tips / suggestions.
