Hey all long time no post!
I have think I have found that certain creatives and headlines work better on particular publishers than others.
For example:
Say you have 2 creatives/headlines and 2 different publishers.
Creative/headline 1. works well with publisher 2. CPA $5.
Creative/headline 2. works well with publisher 1. CPA $7.
But..
Creative/headline 1. with publisher 1 CPA is $10.
and
Creative/headline 2. with publisher 2 CPA is $12.
Is this significant or not and does everyone just use a one size fits all?

It's very common to see campaign elements perform differently per publisher/source.
Sometimes different flows fit certain publisher's websites better than others, other times it happens by chance... But either way I optimize by "one size fits all", and pretty much everyone else does as well.
If the traffic source allows Whitelisting and we're talking about massive volume publishers, it might be worthwhile to seclude these publishers to a separate campaign and optimize its flow separately.

One size fits all is what I'll go with. Unless as you say there's massive volume ( of which there isn't).
We start with 2-3 different text angles and 3-4 images each and have anywhere 10-12 ads per geo
we can see after about a day which ads are the best, and then we pause the rest and run the winners and also upload variants of those
