Everything depends on the offer.
If you run weight watchers or any other bluechip brand, you dont have to cloak. If you run the typical nutra affiliate offers, thats not allowed according to Google's T&Cs so then you would need to cloak.
Same thing for casino/gambling. William Hill, 888casino are legit brands with valid licences and can advertise on google. Shady casinos and advertisers can't.
All ads get reviewed and approved on a case by case basis.

Long version is:
1. Get some web hosting space (preferably in the target country, or use smth like cloudflare).
2. Get some domain names depending on the country and niche you are about to push (or even product name).
3. Get your affilaite manager to send you al the necessary stuff for the API integration.
4. Set it up (some PHP and web hosting knowledge is needed, but not much).
5. Attach a domain to it.
6. Insert the link into the adwords campaign and begin testing.
Optional: tinker with the content of the landing/prelanding page until it's fully compliant with the terms.
