He offers 1 on 1 coaching too...really appreciate any feedback etc

I subscribed to the course for at least a year or so. It became apparent when I implemented the strategies that they weren't working well at all, particularly the recommended method of single Adword/Keyword groupings and also the bid adjustment modifiers, which is a strategy that predates Google's Call Only ads.
I ended up getting an analysis of one of my failed campaigns done by someone running a more up-to-date course and he said that the methods I used were old and no longer required and that I had missed out on several important things. So, I cancelled my subscription that day.
If you are new to Pay Per Call, I would skip Premium23 as the content hasn't been updated for years and you will be following instructions showing the old Adwords interface, etc. It might have been worth paying for a couple of years ago, but not now.
Re. the 1 to 1 service, this isn't something I tried at all, so I really can't comment on that. Just the video content. It would've been good if you were starting Pay Per Call 3-4 years ago but it's all outdated and wasn't properly updated to reflect changes, as far as I was aware at the time.
