Do traffic sources such as Bing or Facebook or any other premium traffic sources ever ban accounts or disapprove ads because of meta refreshes and hide referrers?
Or do they generally not have a problem with it?

Traffic sources don't see referrers.
If your DMR is between your prelander and offer, they won't see that neither.
