Since in these days I have not been receiving a good CTR on my banners I would like to know which guidelines you followed that brought your banners a good click through rate. Colour theory, work on angles, whatever
.
I know that spy tools are another solution but I don't think that success in affiliate marketing comes only from them (since the are suggested for many matters) but also from our own creativity.

It's not possible to give you some universal guide that would suit all verticals, banner sizes etc ... I can however, give you a few pointers regarding things that worked well for ME Personally, so take them for what they are 
1. Short and clear message works better than complicated sentences that result in small letters and hard to read text. Even on large formats, short and simple text always wins for me.
2. A bit of animation usually winds over static presentation. Too much animation can kill the performance on the other hand. So, yes you do want a flashing CTA button, but you don't want the whole banner to look like a flashing Christmas tree.
3. Banners using mild pastel colors in combination with bright/intensive color CTA button worked the best for me. In some cases black&white was the winner. Too much colors on a banner usually resulted in poor performance.
4. Misleading (generic) banners will pretty much always slay legit banners in terms of CTR, thou at the price of a lower CVR. It's something to test anyways. Think about things like play buttons, system messages, message notifications, download buttons, install buttons ... anything that could look like part of a site or an app or a system message. These are not allowed everywhere so check the network rules first.
5. With some banner types (for example adult), the imagery used on the banner has a massive impact. Find the right girl for your banners and the dating leads will flow like mad
Same goes for any banners using some imagery, work on it and get the best possible images. This does not mean the best "quality" images, but the best performing ones - images that your users can relate to or get excited about.
I could go one, but let's start with these few to get the discussion rolling. Anyone else want's to add their experience?
I completely agree with @matuloo on all points, they're spot on and I've noticed the same patterns with banners.
I.e. When it comes to design, look for maximum clarity, good contrast for the CTAs, less text and subtle animations almost always yielded better results for me.
Can most definitely confirm regarding the right imagery with adult banners too - often times most photos would flop and the campaign would have negative ROI, but after testing a whole bunch there was always this one photo that pushed the campaign over to profitability.
Another bit I've played around with quite a lot with is the actual banner shape. I've made variations where the banner has rounded corners, or is rotated 15~ degrees in any direction, the banner background was actually smaller so it doesn't fill the entire 300x250px etc., I've even tested circular ones too. Stuff that would stand out from the rest and attract the eye purely based on the banner's shape, especially if you have a few banner positions on any placement directly above each other.
I'm also a firm believer that a banner doesn't have to be pretty to perform well.
Weird stuff could bring you awesome CTR 
But yeah, the CTRs were probably there
