Can you share a screenshot of the Ad Preview (via Bing Ad Previewer) or from your Search Query? Are both of these being viewed from your Desktop? Can you share the screenshot from your Mobile Device? When viewing actual Ad via your Search Query, was your Ad displayed at the top of the page as the top bidder?
With my limited experience tinkering with Bing Ad Pay Per Call, I don't think it shows an actual button saying 'CALL' on the Ad to end user. Correct me if I'm wrong?
But user should see "Call" option (which they can cancel) AFTER tapping your contextual phone number added as an extension to your ad.
Also as you may already be aware you cannot entirely exclude Desktop/Tablet Search traffic from your Ad BUT if you didn't already you can check-mark the 'Prefer Mobile Devices' option in the particular ads you setup.

Okay strangely enough this now seems to be working. It seemed to take about a day to make this happen. Possibly it was just a delay at Bings end.

Bing ad call extensions do not appear on your ads all of the time. The frequency of the call extensions appearing on your ads is based on ad history, ad relevancy, and ad quality score. If you nail down these three criteria you can get the the call button to appear 99% of the time.
