Landing Page with Mock Questionnaire and GeoIP integration (12)
Hello Fellow STMers,
I built this lander a year ago, ran it successfully for many a months and even still use multiple variations of it.
This is especially useful for the Dating & the Payday Vertical.
It has MaxMind GeoIP integrated - the GeoIP files & integrations are packaged along with the downloadable zip.
The page has a mock questionnaire which prompts the user to interact with the page. This shows details and even their city name and ultimately shows the form, you can replace this with a button of your choice if you want.
The page is extremely light-weight and has been built using Simpliste - http://cssr.ru/simpliste/
Hope some of you find it useful. I'll share more if this takes on.

I just tried it and got the following error
Fatal error: Cannot redeclare geoip_country_code_by_name() in /home/public_html/Private-Dating-Offers/GeoIP-lander-and-database/geo/geoip.inc on line 1593
anyone know how to fix above?

Can anyone provide a similar questionnaire type lander? The one in the OP doesn't work anymore.
