I am going to do The Mobile Appetizer Cookbook but instead of Decisive, am going to focus on Go2Mobi. I hear the traffic quality is better but am wondering if there is a guide around here that goes through the in's and out's of using it.

Good question...would be interested in that as well - I plan on doing the same after I get approved by Go2Mobi.
Go2Mobi is very newbie friendly. My account manager (Mon Gill) did a great job reaching out to me via call once I created my account to address any q's that I had initially. After I ran a campaign or two she offered to have another call to go over anything that I could be doing better.
In short, your account manager is your best viable source there. Just as a heads up, since you are following The Appetiser, they don't have a lot of Mobile wap traffic, most of it is in-app.

App vs Web traffic depends on the exchange. MoPub is the highest volume one where you have mostly App traffic so naturally overall Go2Mobi is more App heavy but you will have website traffic too on other exchanges.
MoPub is 99%+ app traffic. Smaato and Nexage (our next 2 largest supply partners) have roughly 60% app and 40% web traffic. Plenty of mobile web traffic! Depending on where you're running, exchanges like Rubicon and OpenX will also have tons of web traffic,

I had a misunderstanding when I signed up didn't check that I wanted an account manager. But I'd like a contact at Go2mobi. Should I just email support and ask?
Also, I'm waiting for a campaign to be approved now. I noticed placements that I definitely want to run on. If I whitelist them, how does that change the smart bidding? Will my ads run everywhere and the whitelist is given special preference? Or will it only run on the whitelist sites?
You really shouldn't need an AM to get your campaigns going. Their UI is pretty much 1 page and fairly consistent to other DSPs in my experience. I would send their STM account here a PM, I'm sure someone would help though.
Doing a version of the Appetiser for other traffic sources is very much on my to-do list - look for it soon!
