Heres the simple toys r us LP im trying to get through. what can I do to improve it and get it approved? just c&p the disclaimer for the original LP onto mine or... what?

Go to your offer page, and look at the disclaimer below the offer. Usually it would state...
"xxxx.com is not an affiliate with this and that. All prized claimed.... etc etc".... Well, that is your disclaimer. Copy that and paste in your landing page.
An example of disclaimer I am using on my landing page now is:
"WinDailyGadgets.com is an independent rewards program and not associated with any of the above listed merchants or brands. The above listed merchants or brands in no way endorse or sponsor WinDailyGadgets.com's offer and are not liable for any alleged or actual claims related to this offer. The above listed trademarks and service marks are the marks of their respective owners. WinDailyGadgets.com is solely responsible for all Gift fulfillment."
That is it. Once you have updated your landing page, shoot them an email telling them you have made the necessary changes and they will get you approved during their business hour.
Ok will do!

What about for cost per sale stuff, instead of 'freebie sites'. Does anyone have an example of what you could use (i.e the minimum you can get away with) when you promote an actual product as an affiliate?
Also, can you just hyperlink to it 'above the fold' .... meaning you just put 'Disclaimer' as a link, and then put the affiliate info on a separate page, ... or does it actually have to be pasted out in full on the page they've flagged as requiring it?

Honestly I just put a disclaimer above the fold in big font while they approve it.
After it gets approved I remove it completely.
Never had any problems.

LI change their tune regularly about the disclaimer they are ok with.
On numerous occasions I mailed back immediately asking for an example of a suitable disclaimer and sometimes I get a response with something I can use.
Ive not had the decline reason you included before so not sure what they would be happy with.
Maybe just something like
"Advertorial" at the top
or
"This website is promoting product X. The product advertised is the property of company Y. This website is not not associated with any of the above listed merchants or brands."
