You need to apply all 3. Have a look at the type of banners and landers that are running for similar offers.
Have a look around - other networks may have a better street rate on that offer. Get pay bumps asap
You need to focus on better creatives to get higher CTRs. The landing page is pretty good, but I've seen better, nothing wrong with seeing what is out there for inspiration. Also, need to find the right pockets of inventory.
The two most important things I've found is GEO + Site ID

That lander has 26 words, and you are getting 18 cents for the lead. Change one of those numbers.

do you think it is possible for that payout to work? what cpms are you paying? what conversion rate do you think is reasonable to expect?
you need almost 2 conversions per mille to make money at those cpms. that's difficult. not impossible. but difficult.
here are some major variables that can greatly affect cpa
1. offer (du speed du battery lazy swipe)
2. affiliate network
3. angle (you already know this)
4. bid - usually lower is better, try to find out whats the average bid for the exhange and geo and you can usually afford to bid lower than that.
5. country choice
