Ok, my first follow-along ...
My AM put me on to a few offers from the same advertiser that are SOI that he said were converting well.
I thought I'd put my coding chops to use and do a survey lander with an offer wall where the users can pick their "prize".

Anyhow, after a few hours today, I was able to get some decent CTR on my banners but absolutely brutal CTR on my lander.
Here's my Decisive stats (I'm happy with the CPC and the CTR - without any optimization to the banners - yet)

So, I'm excited so far - there's room for improvement on the banners, but so far so good.
My lander is pretty basic (screenshots below) - 2 pages (technically one - using jquery mobile - two pages one file). Page one is the welcome and the survey. Page two is the offerwall.

Everything works smoothly and quickly, but my banner CTR is brutal (less than 1% at this point).

I'm at my wit's end at why this isn't at least converting into clicks to the offer let alone conversions. I'm thankful the clicks are so cheap so I can accumulate more data.
My thoughts (and I'm probably over thinking things):
Oh, as a sanity test, I also direct linked the offer with a little less traffic maybe 650 clicks and no conversions.
My AM assures me the offer is converting and likes my lander - but in this instance (where LP CTR isn't a factor), no joy (conversions)
Improve your LP, look at your page and ask yourself if its good mobile flow - second step is mobile optimized, but first?? hell no.
Make few steps with one question at step to make it look like step two on each before ( I didnt mean the same design, but same optimization ).
Try to spy on your competition and look what works for them
