Budget is set and ads / ad-sets are both live. funding is attached to the account. all ads are approved. but they are not running even a single impression or dollar of spend.
has anyone seen this before? is the account soft banned or is it just a simple issue i'm overlooking?
No, this is normal with fresh accounts. For your first website clicks ads it takes a couple of hours before you get impressions, because your account goes through a manual review first nowadays. So I hope your ads are clean!

Delete the clean ad and make new clean ads?
I would test the waters by trying to force some clean campaigns through - although not charity campaigns and crap, rather brand building for a page or product. Something that looks like it has a purpose other than getting ads approved.
Sometimes if you set it as 'start from now' then it gets stuck like that. Happened to me all the time. I actually schedule ads to start about .5-1 hrs after current time, (check what timezone your account is running at).
Try that, see if it fixes, if not contact their support, they actually DO get back to you believe it or not.
Anyone have ads running for a couple days then it stops spending ?
When this happens to me I usually keep submitting more and more clean ads and watch the account until it starts spending. Sometime it takes 3 new campaigns or more! Try to submit ads for Page Like and Newsfeed Post Boosts.
Just be patient and don't give up!
Also it is useful to build up some quality score within your account before you go for "the kind of ads you're trying to run"... Make sure your ads' have a high CTR and good engagement rate!

OK doing that right now , been having these a lot so wondering if anyone knows what actually causes the manual reviews - budget increases, hitting specific spend or new billing threshold, targeting, or keywords in ad copy etc. that trigger it
Submit your ads... Wait 2-3 hours... Account not spending yet? Submit a new set of ads! Make sure you do at least 3 ads per ads set.
You account should start spending eventually so just keep a close eye on it.
Manual reviews may depend on a lot of factors (keywords, user flaggings, reaching a new billing threshold), but when you set up a new account and submit your first ads it is very unpredictable! Some accounts will be on auto-approve and some will take time to get approved and start running.
