So this is what I did:
1. Picked a CPA offer (nothing similar to "risky" weight loss or biz op type).
2. Configured my cpvlab with three different affiliate offers (no LP's, just direct links)
2. Added some banner creatives to bonadza, got them approved.
3. Bid on 20-30 websites (bids vary from 1.00 to 6.40).
The problem is I can't get any impressions.
According to a helpful bonadza manager:
By aggressive they probably mean to add a lot more placements.
Did you just add high volume ones? Just add a lot more placements and bid aggressively but with a low daily budget, that way you will find out where you are getting impressions.
Desktop display CPMs can be astronomical. A $20 CPM on some high volume US sites is not uncommon.
Another approach would be to add the top 100-200 placements in a specific category or across the board and bid low, e.g. $0.50 CPM.
That way you may find cheap traffic sources. You can bump bids systematically and explore the inventory from bottom to top.
I now have 111 traffic sources. Still 0 impressions. The bids vary from $1 to $20.5. Should I try raising the $1 bids one by one? By how much should I start raising them $0.5-$1? What should be my default bid I should start with?
150 various traffic sources with various bids and still nothing...
Sanity check: are all your ads approved?
I would set the campaign budget to say $10 and then raise the CPM bid to $20 on everything.
If you get no impressions, just talk to support again.

You should be far more aggressive and use your campaign settings to your advantage.
Set your daily budget to $10 or whatever the minimum is.
Now raise your CPM bids to $20 on every single placement.
Your problem is that you are not getting impressions. So change your test conditions so that you absolutely should get impressions, with a safety net to stop excessive spend.
If you still don't get impressions, it will be a technical issue of some kind.
It's also much more convincing to a support team when you tell them "hey, I have 10 creatives that are all approved in campaign X. I am targeting US traffic, above the fold, no other targeting. I have bid $20 CPM on 100 placements and am running a daily budget of $10 but still have zero impressions. Please investigate".

$20 def. bid on 150 placements did not help. Still 0. I am really disappointed with bonadza. Looking forward to what the manager will have to say about this.
Asking the obvious here, but still, have you considered a different traffic source?
That makes sense and explains the behaviour you saw.
The blacklist they are using will probably have nothing to do with SiteScout and could be from the exchange(s) they plug into and agreements with pubs to maintain quality.
It's a good thing really that they have that, it means a more level playing field and higher quality pubs stay on.
What was the offer? Is your tracking domain old? Check both on MyWoT and Google e.g. "offer name scam" to see if there is consensus. If it's your typical crappy affiliate offer, it wouldn't be surprising that it was blocked.
