That's a tonne of clicks from which to get only one conversion...
Banner CTR seems fine. Do you mention the app in the banner?
It could be that the offer is saturated but I doubt that would give this kind of conversion rate.
I posit:
a) the offer is a dud
b) there is an issue with tracking somewhere = conversions not registering
c) there is something else wrong out in cyberspace
Talk to your AM and get feedback and otherwise move on from the offer. It happens and it's best not to decrease productivity by dwelling on it.
Hmm, yeah, that's not a great sign.
Given just how low the payout is, I'd recommend pausing traffic on all campaigns that have spent over 7 dollars. Leave everything else running, just in case, but pause those things at 7 dollars too.
It's possible it only converts well with one particular wifi/app/site/carrier/country combo, so worth checking for that, but on the campaigns that have semi-decent spend already I don't think you'll lose by assuming the offer won't convert there.

I actually have been in contact with my AM... and she said the offer is one of their top3 mobile app-ins...
and today... the offer requested to stop the specific country... (I think this is because they already got a lot of install from this country?)... I was also promoting on this country...
I did what Caurmen said in the cookbook... to ask my AM about specific targeting... and I got the country/ISP/mobile model... and I started with those... so basically in other guys this offer is performing well... because the offer requested to stop traffic from the specific country...
I thought I got the good stuff... coz I got a specific targeting from my AM.. and I got the country and mobile model... The only difference is my banner... but it looks like the banner got good CTR as well... might be my adcopy?

there's a lot of junk traffic in targeting: "SITE" (I just can't mention what site)... I saw what mrgreen said about having "sitewide/publisher blacklisting" and I agree with him...
because if I blacklist a wifi/site or mobile/site (SITE)... in one camp... this certain sites... starts appearing/eating impressions in my other camps... so I have to blacklist it again in that camp...
I believe I got around 75% clicks/impressions from wifi/site and mobile/site... their CPC is very low though.. $0.01? but probably because they don't convert..
Well, if you targeted a specific model/country that has been doing well (according to AM), chances are it was quite a competitive pool already, and if your banners didn't have the app name, there's a chance people have already clicked on ads for the app before.
Still, there could be many factors contributing to this outcome.
