Wondering who has had experience with ad auto-approvals when launching a new campaign on an account that has been running compliant traffic. Do you feel there is a certain dollar amount (total lifetime spend) where they typically start pushing ads through without review?
They auto approve creatives all the time when they use previously approved images, URLs, etc. It varies with accounts, geo's, etc. though. Not sure if there is any specific pattern.
I can say with some confidence that I don't think the ad budget has anything to do with approval at all, that would be a dumb loophole for them to have. Or do you mean historic account spend?

and yes, I meant historic spend. would be lovely if we could just set the daily spend to 5k and have them close their eyes 
If i can add on. Does anyone know just how much you can spend on a personal account with the 750$ threshold. I've heard different things. Some say don't spend 2-3x's more then threshold, or you can spend 1500 after the first day, and 5k so on. Last week I increased the daily spend from 1k to 4k overnight after a couple days, and the next afternoon, got the ban. Currently am spending a little over 1k daily steady. Seeing how far you can push it
. Anyone? I want to try again, but maybe knows something I don't.
Are you asking how long you can run dodgy stuff before getting caught due to spend increases?
AFAIK, if your threshold is $750 it's likely that your daily limit is $1500. However I'm not sure if they actually state the daily limit for you in the ad manager.
you can spend 5k on a $750 threshold...but you are basically always going to get your account axed when you increase your daily budget 4x running non-compliant stuff.
The threshold is more FBs way of saying:
"we trust your payment source enough to give you a little extra breathing room on transaction frequency"
not
"we trust your account enough to let you spend $X,XXX daily"
you need secure or trusted payment source.
