Payment models offered by affiliate networks

Hey all.
So here's my very first newbie post. When I look at the registration forms that aff networks present upon applying for their services, some of their questions aren't 100% clear to me. For instance, on adsimilis they request what payment method you choose (cpc, cpm, cpa,...). This is from me to them (this is my guess but I would like to be certain)? Or their payout to me?
Also, my level of profesionalism ("corporate" website etc.) is currently a work in process. I have some html skill so that isn't a problem. But should I heed too suggestive url's eg. golddigger.net, makeitrain.com, *insert whatever*.com,...? I already found the best practices for applying to the afffiliate networks using search, after my first application :'(, with f5 media so I'll see how that one pans out.

I'd love to hear your thoughts and experiences on these murmurings of mine. So let it come.

If they are asking about payment methods like that it implies you are trying to register as an advertiser (i.e. offer owner), not an affiliate/publisher.

Thanks for the quick reply. You had me doubting so I went back to check. They effectively ask this on the affiliate registration form:
