On asking for payout increases...

If you're crushing an offer, what % increase in payout do you usually ask for?

Do you usually go to your AM, or to the company promoting the offer to cut out the middle man?

Do you ask for a different % increase depending on whether you're pushing high volume/questionable quality vs lower volume/high quality?

Much appreciated friends

It all depends on 2 factors :

1 - Volume you are doing
2 - How much you can float

If you are doing big volume and floating money is not a problem ( since most advertisers don't just pay weekly ) , I would say go direct.

Otherwise just hit your AM and ask what's the max bump they can give you for the current volume.

Sometimes makes sense to justify it as well. If it's going to allow you to scale more then just say at x I'm going to be able to run y by bidding more. Which will mean and extra $z based on the added volume.

You can always just ask as well of course. Come with how it's a win/win for everyone and you'll get more usually.
