I currently use my private fb account for running ads. If I hypothetically would get banned for doing some cloaking or whatever, would my whole account get shut down?
No, you just can't run ads any more.
You will still be able to use the account but you will lose access to the advertising functionality of the account. You will also no longer be able to make any type of payments on FB. This includes payments in games, apps, ads etc. But you will still be able to access your profile and use it socially.

You're unlikely to lose your personal account for it... but be aware that Facebook can do whatever they want. You are never 100% safe.

Should we just always use a separate Facebook account under a fake name for running Facebook ads if we're in any doubt as to whether they could incur a ban?

Yes, just don't run ads on your personal account. They are unlikely to ban your personal account because of a ban on a separate account should they link them (which is unlikely if you're not trying to advertise on the personal account).
