W8-BEN and SS-4 forms to prevent tax withholding

Could someone share their experiences with W8-BEN and SS-4 forms? Do I need to get the EIN to fill W8-BEN if setup as a company?

And if I do need EIN, for line 9a in SS-4, for Corporation, there's a need for form number for tax filing. Does it mean that I need to file tax every year in US? Or can I put N/A there?
