Anyone good with forms?

I want to make an email form disappear once user fills out name and email. How can i do this?

I'm using an Aweber form, so I'm just copy and pasting their one line javascript code.

You'd need to add some jquery onto the page to do that provided that it wont fuck with aweber's integration code.

If you're not technical any monkey on odesk should be able to sort that out for you. Or email aweber support...
