So after jumping through a few hoops (including the creation of a fresh account), I finally got my campaign approved. Yay! Let's get cracking!
Unfortunately I've already run into a brick wall, because for some reason my ad is not getting any impressions. I am bidding in the upper range of the recommended CPM and have also tried an optimised CPM bid set to break even values (CPC bid = listed EPC on offer, cost per action = conversion value of offer) on another campaign. I'm now 24 hours in and still 0 impressions, 0 clicks, 0 actions.
My target market is 3 million people.
What's going on?
I had this same issue before, apparently you should not follow the recommended bid. This bid is usually set within the advertisers in your ad network, they do not include advertisers outside. So you might have to outbid them.

Hmm the recommended bid has drastically changed. Now that I'm bidding in this new (obviously higher) zone, all seems to be running. How are these bids determined?
I can;t make any sense of this. Some of my ads within the same campaign (same demographic, same headline, just a different image) get a different suggested bid than others. Why is this?

Yes it is, there are many threads that deal with this. NEVER pay attention to minimum bid. Ever. Just bid high. Later you can taper down if you want (I never do, my bid is always 50-100x what my CPC is)

Your goal is to get crazy CTR, so bid high until you see some winning ads, then you can lower your bids and your best ads will remain getting clicks at the lower bid
You reckon I should just follow facebook's lead and bid $40 CPA on optimised CPM? I like the idea of optimised CPM, since you can bid on EPC and the conversion value of an offer.
