Has anyone made PPV work for niches outside competitions, ipad offers, rebills, etc.?

Yes, but the quality is almost always very bad, so that's why it tends to be used for "low quality" and low pay offers. There are exceptions to this rule, but *in general* the quality of PPV is very poor.
I once got it profitable on PPV dating (Christian dating,Jewish dating) but, like joshogle said, I got kicked off the offers for lead quality issues. I did everything possible to improve the lead quality but nothing helped. One network even kicked me out all together and I was doing nothing shady. Many of the people on PPV are just poor IMO.
That being said, I know people doing $500-$1000/day on PPV dating, insurance and other offers. Also, many people seem to agree (including me) that LeadImpact's traffic quality took a nose-dive around last November. Many people making money on LeadImpact slowed down to a crawl around November. Since LeadImpact and Traffic Vance are both $1k deposit, go through the trouble of getting a Traffic Vance referral if you are intent on doing PPV. However, its a slowly dieing traffic source IMO.
I have only ever faced quality issues on some dating offers.
I have been running financial offers, payday offers, dating offers, credit card offers, debt offers, revenue share offers and whatever else I can test now for more then 6 months and they have all done nicely.
There is a lot of saturated targets on ppv but if you work hard enough at doing your homework and coming up with low cost targets you can do really well. I find 90% of targets people bid on are the top 10 results in google and are not very creative.
It is a low purchase intent channel (for obvious reasons).
