I recently opened an ads account on a 4 year old account. The first ad was for creating page likes which immediately afterwards i was banned. CC matched name on account and was created from the same computer which was always used. I've contacted fb several times and get the same response.
"Our records indicate that your payments account was disabled due to suspected fraudulent activity. In the interest of protecting our users, we will not be able to reenable your payments account. This decision is final.

I live in Rotterdam and Al Hoceima. I had an account running for a couple of years, but like you I didn't touch it for a year or so.. Normally I log in on my computer in Rotterdam, but when I started again using my Facebook Ads-account I was in Al Hoceima. They immediately banned my account, because of the same reason as you..
I got the same message from that Bob, created a new account with a CC (I never used my CC before on Facebook, I used PayPal in the past) and since then didn't get problems anymore..
So I advise you to make a new Ads-account, use a new credit card or an PayPal account and go further... If you want to do it even better use also a new IP address..
Bob is a dick. You also need a new address and name...
Just a thought, but has anyone considered vetting their CC by adding it as a normal user would and wasting money on e.g. Farmville first? I'm sure FB doesn't want to disable accounts of their users who want to waste money on FB apps... If you're not doing anything wrong ads-wise then perhaps this is a safer way to get the ball rolling... No idea if it will have an impact but it can't hurt to have a few weeks of FB happily charging the CC first.
As you keep doing more on Facebook you'll learn to hate Bob more and more.
Fortunately, if you didn't do anything shady, there's a fair chance you'd get the account back if you keep emailing them.
